PRN LPN Nurse – 12-Hour Shifts (Day & Night)

Schedule: PRN – As Needed | 12-hour shifts

• Day Shift: 6:00 AM – 6:30 PM

• Night Shift: 6:30 PM – 6:00 AM

• 40 hours per week not guaranteed

Position Overview

Provide professional nursing care, treatments, and medication administration for residents in a geriatric/long-term care setting. Collaborate with the interdisciplinary team to ensure residents receive quality care and support.

Key Responsibilities

• Administer medications and perform treatments per physician orders (oral, enteral, parenteral, suppository; wound care; tracheostomy, gastrostomy, jejunostomy, catheter care).

• Observe and report effectiveness of medications and treatments.

• Maintain adequate medical supplies and medications.

• Communicate resident needs and care updates to staff, residents, and families.

• Participate in shift reports and maintain accurate clinical documentation.

• Complete admission assessments and assist with Minimum Data Set (MDS) assessments, care plans, and quarterly reviews.

• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and staff meetings.

Qualifications & Skills

• Knowledge of gerontological/geriatric nursing theory, aging processes, pharmacology, and team nursing practices.

• Ability to observe, assess, and report resident behavior and progress.

• Skilled in implementing care plans and applying state/federal nursing regulations.

• Strong oral and written communication, interpersonal, and professional problem-solving skills.

Abilities

• Follow nursing regulations and protocols.

• Respond appropriately to medical emergencies.

• Evaluate and analyze client health issues; make independent decisions.

• Maintain professional relationships with residents, families, and staff.
