Position: PRN LPN – Flagstaff, AZ
Pay: $40–$44/hour
Schedule: 12-hour shifts (6:00 pm – 6:30 am), PRN, must work at least 3 shifts/month, every other weekend off

Responsibilities:

• Administer medications and treatments per physician orders (oral, enteral, parenteral, suppository, wound care, trach/gastro care, catheter care)

• Observe and report effectiveness of medications and treatments

• Participate in shift reports and maintain adequate supplies

• Communicate with residents, families, and staff

• Complete admissions, care planning, and documentation (MDS, quarterly reviews)

• Participate in QAPI and staff meetings

Required Documents :

• Active LPN license

• CPR/BLS certification

• Level 1 Fingerprint Clearance Card

• OIG & APS clearance

• 2-Step TB Test

• Hepatitis B vaccination or signed declination

• Vaccine Status (including COVID-19)

• Resume
