Position Summary: The Re-Entry Specialist position is responsible for case management responsibilities and ensuring member satisfaction by working collaboratively with cross-functional clinical and peer support teams to assess member needs, evaluate transitional service options, create transition plans, coordinate care, and monitor progress. For adults with recovery and re-entry issues, recovery and success in life depends on good mental and physical health, as well as having satisfying relationships, safe housing, purpose in life, a sense of belonging, and hope for the future. This includes assisting the members with acquiring and utilizing necessary skills to become self-sufficient and to not only effectively manage but also enjoy their lives.
Qualifications and Requirements
● Meets the requirements of a Behavioral Health Technician
● Must be 21 years or older.
● High school diploma or GED required
● CPR/FA certification required
● Crisis de-escalation certification (i.e., Safety Care, CPI, etc.) required
● Fingerprint Clearance Card
● 2 Step Tuberculosis Screen and Risk Assesment
●Physical Attestation / Physician completed Physical
● Intermediate computer skills – MS Office (Excel, Word, Outlook, Teams), Internet, Electronic Health Record, etc).
● Excellent communication, organizational, time management, and critical thinking and problem-solving skills required.
● May require driving and operation of a motor vehicle, as needed
