Registered Nurse (RN) – Flagstaff Area

Pay Range: $52 – $54/hour
Schedule: Flexible shifts available

• 7:00 AM – 3:30 PM

• 8:30 AM – 4:30 PM
  Final schedule to be determined by the Administrator. Overtime may be required.

Qualifications

• Active RN license in the state of Arizona

• Previous experience in a long-term care setting (required)

Required Compliance Documentation

• Updated professional resume

• Active Level 1 Fingerprint Clearance Card

• OIG/SAM name clearance check

• Current CPR/BLS certification

• Completed 2 step TB skin test

• Valid RN license/certification

• Proof of Hepatitis B vaccination or signed declination form

• Proof of COVID-19 vaccination or signed declination form

• Completed Agency Skills Assessment Packet
