Wound Care Nurse – RN or LPN 

Schedule: Flexible | 8-hour shifts, Monday–Friday (No weekends)

• Options: 7:00 AM–3:00 PM, 8:00 AM–4:00 PM, or 8:00 AM–5:00 PM

Position Overview

Provide specialized wound, ostomy, and continence care for residents in a skilled nursing or long-term care setting. Serve as a clinical resource for staff, ensuring residents receive effective care and education.

Key Responsibilities

• Perform wound assessments, reassessments, and management per policies and procedures.

• Develop skin care programs to prevent breakdown.

• Implement and revise care plans for residents with wounds, fistulae, ostomies, or incontinence.

• Assess and manage pain; educate residents and families on pain management.

• Serve as a resource for nursing staff, providing in-services on wound prevention and care.

• Communicate clearly with nursing staff and physicians; consult with healthcare professionals for interdisciplinary care.

• Ensure documentation meets current standards; attend continuing education to stay current in wound care practices.

Requirements

• Active RN or LPN license in Arizona

• Certification as a Wound, Ostomy, and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN)

• Arizona Fingerprint Clearance Card (or ability to obtain)

• Current CPR/BLS certification

Preferred

• Experience in a skilled nursing facility or long-term care
